I hereby give permission for the Team Manager to give the immediate necessary authority on my behalf for any medical or surgical treatment recommended by competent medical authorities, where it would be contrary to my son/daughters interest, in the doctors medical opinion, for any delay to be incurred by seeking my personal consent.

Photographs – I give my consent to Swim England South East for photographs to be used for marketing/publications on the Swim England South East website, social media accounts and newsletters in line with Wavepower 2000-2023.

I am in good health and injury free and can take a full and active part, I will notify the coaches on the day should this change.

Data Protection – I give my explicit consent to Swim England South East processing my sensitive personal information as a competitor in order to take part in an event or competition in line with the Competitor Data Privacy Notice. We take your privacy seriously and will only use your personal information in accordance with our Privacy Policy.
